Death Certificates, 1912-1939 (Accession 36390)

This series documents deaths that occurred in the Commonwealth, 1912-1939. These death certificates certify the facts of death and were required to be filed with the State Registrar of Vital Records.

These certificates are arranged chronologically by year and thereunder by month. For each month the certificates are arranged alphabetically by county or city. Certificates are numbered consecutively for each year. Prior to 1932, cities are filed with the counties in which they are located. Beginning in 1932, cities and counties are filed separately, with cities at the beginning of each month. At the end of each year there is a volume that includes delayed filings for that year.

Use Tips:

An index to these death certificates is also available on microfilm (Accession 36512). It is important to obtain the year of the death and the certificate number from the index. This will allow you to use the in-house finding aid to identify the correct reel number on which to locate the certificate.

The index is arranged chronologically by year groups (1912-1914, 1915-1919, 1920-1924, 1925-1929, 1930-1934, 1935-1939, 1940-1944, 1945-1949, 1950, 1951, 1952, 1953, and 1954), and thereunder alphabetically by surname.

The first segment of Index Reel 1 consists of a death index that was created from transportation permits for deaths prior to 1912 and from 1912-1927. We do not have the records that correspond to this index, although the index may be helpful for establishing death dates, especially for those deaths that occurred prior to 1912. Death certificates for the years 1896-1912 were not filed with the Bureau of Vital Statistics.
